Goldfinch Health Dental Program.

Goldfinch’s Dental Program provides artists financial support towards preventative and restorative dental care. Members will have access to up to a $50 grant each year in the form of a reimbursement.

This will cover public health dental procedures (including annual check ups and cleanings) aimed to promote oral health and prevent disease.
Cosmetic procedures are not covered.
Any remaining dollars not used by the member will expire at the end of the 12-month membership period.
The grant is only applicable to procedures completed during the current membership period.

Please fill out the below form after receiving treatment, and you must submit proof of procedure/receipt in order to be reimbursed. Submissions that do not comply to the above guidelines are not subject to reimbursement.

Please provide a brief description of the completed procedure. This will be used to verify eligibility.

Acknowledgement

Please email admin@mygoldfinch.org with the subject line "Dental Reimbursement Application" containing evidence of described procedure. This can be a receipt or invoice, and must show the procedure name, date, amount billed. Note that your application is not complete until we receive this email.
